Home Care LINK is a web-based service that records the actions and observations of home caregivers. Home Care LINK uses mobile devices to create a digital connection between the care coordinator and the caregivers. It includes time and attendance, GPS, pictures, and timely reminders.
The safest, best place for people to stay — as long as they have care — is at home. Home Care LINK allows loved ones to stay home and avoid the disorientation and depression that comes with sending care recipients to the hospital or a nursing home.
Home Care LINK provides peace of mind for people with loved ones receiving home care by sending regular updates on the loved one’s status, immediate alerts for critical situations, alerts if the caregiver does not arrive as scheduled, and records medical providers’ required home measurements, such as temperature or blood sugar. With Home Care LINK, caregivers are also able to take pictures of the loved one; for example, a picture of a wound or injury they have, or a receipt, or simply take a picture of the smiling loved one.
Home Care LINK permits the Care Coordinator to have a much more timely insight into the health condition of their loved one. As a result they are able to react to changes in conditions, and in many cases prevent ER visits and hospitalizations. This service has the potential to improve health outcomes and reduce medical costs.
JB Holeman developed Home Care LINK after becoming frustrated with lack of communication when his own elderly mother was in the care of doctors and nurses. Holeman was responsible for managing the care of his elderly mother from a different state during the last few years of her life. The task represented many additional struggles beyond just the emotional burden of watching a loved one age. He was forced to rely on “hope” that everyone was doing what they were supposed to do, because his mom was not always the most reliable source of confirmation. Holeman found that getting quick, accurate answers was a struggle.
Home Care LINK is especially helpful for people who have loved ones with chronic disease conditions that live miles away. The distance makes it impossible for the loved ones to be there for every appointment or accurately keep tabs on them. Home Care LINK bridges that gap with its recording and monitoring tools. Now the Care Coordinator is able to react much more quickly to resolve issues before they become a crisis.
