Operator error can range from distraction, cognitive errors, failure to follow safety operation rules.  On a busy manufacturing plant floor there is often poor supervision of operators, inexperienced operators, and unstable loads.

Inexperience and limited training driving ramps, rough terrain are only part of the problem.  The rules of road in a manufacturing facility do not favor pedestrians, nor are plants designed to avoid blind spots, narrow aisles, building columns, and tight turning radii.

Fork truck free initiatives are increasingly popular as sufficient data has revealed the true cost of fork truck ownership. Where purchasing managers are focused on lowest cost acquisition, operations managers must account for the extensive budgetary requirements in maintaining the fork truck fleet.
