Hyperkalaemia: An Overview

Hyperkalemia is defined as a high potassium level in the blood. Potassium helps in the proper functioning of nerves and muscles, including the heart, but too much potassium in the blood can be dangerous and can cause serious heart problems and sudden death.

Hyperkalemia is defined as a serum potassium concentration that is equal to or greater than 5.5 mEq/L from a non-hemolyzed whole blood sample. The normal potassium level in the blood is 3.5-5.0 mEq/L. Furthermore, it can be classified according to serum potassium into mild (5.5–6.5 mmol/L), moderate (6.5–7.5 mmol/L) and severe (>7.5 mmol/L) hyperkalemia. Often hyperkalemia is asymptomatic, but at times they are often nonspecific such as heart palpitations, nausea, weakness, or paresthesia. Common acute manifestations of significant hyperkalemia include muscle weakness and ECG changes, with the latter having the potential to progress to a life-threatening arrhythmia.

Risk factors for hyperkalemia include several conditions such as chronic kidney disease, diabetes, congestive heart failure, and certain medications that disrupt the potassium balance. Also, males are found to suffer more than females with Hyperkalemia.

Hyperkalaemia Market Key Facts

  • In 2021, the total diagnosed prevalent cases of Hyperkalemia were estimated to be approximately 5,517,000+ cases in the 7MM. These cases are expected to increase by 2032 at a CAGR of 2.8% during the study period (2019–2032).

Hyperkalaemia Therapeutics Analysis

The treatment of hyperkalemia is directly related to the swiftness with which hyperkalemia has developed in an individual with the absolute level of hyperkalemia and the evidence of toxicity. The faster the potassium rises, the higher the level is, and the stronger will be the evidence of cardiotoxicity, resulting in the requirement for more aggressive therapy.

However, frequently used therapies for hyperkalemia may also be associated with morbidity. Therapeutics may include the simultaneous administration of insulin and glucose (associated with frequent dysglycemic complications), β-2 agonists (associated with potential cardiac ischemia and arrhythmias), hypertonic sodium bicarbonate infusion in the acidotic patient (representing a large hypertonic sodium load) and renal replacement therapy (effective but invasive). In addition, potassium-lowering drugs can cause a rapid decrease in serum potassium levels, leading to cardiac hyperexcitability and rhythm disorders.

To further improve the treatment outlook, several major pharma and biotech companies are actively working in the Hyperkalaemia Therapeutics Market.

Systemic Lupus Erythematosus: An Overview

According to the American College of Rheumatology, Systemic lupus erythematosus, referred to as SLE is a chronic (long-term) disease that causes systemic inflammation that affects multiple organs. In addition to affecting the skin and joints, it can affect other organs in the body such as the kidneys, the tissue lining the lungs (pleura), the heart (pericardium), and the brain. Many patients experience fatigue, weight loss, and fever. The times when symptoms get worse are called relapses, or flares. The times when symptoms are under control are called remissions. The general symptoms are not specific. Lupus flares vary from mild to serious. SLE is most commonly seen in women in the reproductive age group (frequently starting at childbearing age), although lupus is increasingly recognized after the age of 40 years, particularly in Europeans.

The Lupus Foundation of America (2016) estimates that 1.5 million Americans, and at least five million people worldwide, have a form of lupus, and systemic lupus accounts for approximately 70% of all cases. Non-Hodgkin lymphoma is a type of cancer that generally develops in the lymph nodes and lymphatic tissue found in organs such as the stomach, intestines, or skin. In some cases, NHL involves bone marrow and blood. NHL is not just one disease–it’s actually a diverse group of blood cancers that all arise from lymphocytes (white blood cells that are part of the immune system). Lymphoma cells may develop in just one place or in many sites in the body. NHL has many different subtypes, which are either indolent (slow-growing) or aggressive (fast-growing).

Non-Hodgkin lymphoma is divided into more than 30 types, classified based on the type of lymphocyte involved: B lymphocytes (B cells) or T lymphocytes (T cells). Non-Hodgkin lymphoma is further classified by other factors, including whether it is aggressive (fast-growing) or indolent (slow-growing).

As per the American Cancer Society (2022), NHL is one of the most common cancers in the United States, accounting for about 4% of all cancers. About 80,400+ people are anticipated to be diagnosed with NHL in 2022.

It also evaluates the current Osteoporosis treatment practice/algorithm, key drivers & barriers impacting the market growth, and unmet medical needs to curate the best of the opportunities and assess the underlying potential of the market.

Osteoporosis: An Overview

Osteoporosis is defined as a systemic skeletal disease characterized by low bone mass and microarchitectural deterioration of bone tissue, with a consequent increase in bone fragility and susceptibility to fracture. This well-established definition, developed by the international consensus in 1993, captures two important characteristics of the disease: its adverse effects on bone mass and microstructure, and the clinical outcome of fracture.

It is a condition that often remains undiagnosed until it manifests as a low-trauma fracture of the hip, spine, proximal humerus, pelvis, and/or wrist, which frequently leads to hospitalization. Estrogen is considered important in maintaining bone density in women. When estrogen levels drop after menopause, loss of bone density accelerates. During the first 5 to 10 years after menopause, women can suffer up to 2-4% loss of bone density per year.

The causes of osteoporosis include hyperparathyroidism, hyperglycemia, medications, smoking, alcohol use, oxidative stress, inflammation, calcium deficiency, menopause, etc. The most common symptom of osteoporosis is a vertebral compression fracture or hip fracture. The compression fractures in the spine, caused by weakened vertebrae can lead to pain in the mid-back area. The fractures often stabilize on their own, and the pain goes away, but sometimes the pain persists because the crushed bone continues to move around and break. The symptoms include loss of height, back pain, increased number of bed days, depression, etc.

The key risk factors for osteoporosis include genetics, lack of exercise, lack of calcium and vitamin D, personal history of fracture as an adult, cigarette smoking, excessive alcohol consumption, history of rheumatoid arthritis, low body weight, and family history of osteoporosis.

Osteoporosis Market Key Facts

  • According to the CDC, in the US, the prevalence of low bone mass, a precursor of Osteoporosis, at either the femur neck or lumbar spine or both among adults aged 50 and over was 43.1% and was higher among women (51.5%) compared with men (33.5%). Osteoporosis prevalence increased from 2007–2008 to 2017–2018 among women, but not men.

  • According to the International Osteoporosis Foundation, in the UK, the prevalence of Osteoporosis in the total population amounted to 5.2%, on par with the EU27+2 average (5.6%). In the UK, 21.9% of women and 6.7% of men aged 50 years or more were estimated to have Osteoporosis. Hip fracture is the most serious consequence of Osteoporosis in terms of morbidity, mortality, and health care expenditure.

  • According to a study conducted by Bolster et al. titled “Osteoporosis” states that about 8 million women and 2 million men in the United States have osteoporosis. In addition, more than 95% of women and about 80% of men are primary osteoporosis. Whereas, secondary osteoporosis is less than 5% in women and about 20% in men.

  • DARBA et al. conducted a study titled “Cost-utility of denosumab for the treatment of postmenopausal osteoporosis in Spain,” which found the prevalence rate of postmenopausal osteoporosis (PMO) of approximately 25% for women aged 60–69 years and 40% for those aged 70–79 years was in Spain.

Osteoporosis Therapeutics Analysis

The treatment of osteoporosis involves treating and preventing fractures and using medicines to strengthen the weak bones. According to the International Osteoporosis Foundation, for patients at high risk, drug treatments are needed to effectively reduce the risk of broken bones due to osteoporosis. Treatments have been shown to reduce the risk of hip fracture by up to 40%, vertebral fractures by 30-70%, and, with some medications, reduce the risk of non-vertebral fractures by 15-20%.

The treatment landscape of osteoporosis comprises mostly pharmacological therapies, which is further characterized as anti-resorptive agents (i.e., bisphosphonates, estrogen agonist/antagonists (EAAs), estrogens, calcitonin, and denosumab), anabolic agents (i.e., teriparatide and abaloparatide), and mixed agents (i.e., Romosozumab). Apart from the mentioned pharmacotherapies, there are several other anti-resorptive treatment options to treat osteoporosis, such as estrogen replacement and selective estrogen receptor modulators, which further comprise of raloxifene, lasofoxifene, and bazedoxifene. In addition, a few dietary supplements such as calcium and vitamin D also provide benefits to patients with osteoporosis.

Transthyretin Amyloid Cardiomyopathy (ATTR-CM): An Overview

Transthyretin Amyloidosis (ATTR) is a slowly progressive condition characterized by the buildup of abnormal deposits of a protein called amyloid (amyloidosis) in the body’s organs and tissues. ATTR amyloidosis can impact numerous organs and tissues in the body, including the peripheral nervous system, and organs such as the heart, kidneys, gastrointestinal tract, and eyes.

According to the World Heart Federation, transthyretin amyloid cardiomyopathy (ATTR-CM) is a presentation of ATTR amyloidosis that primarily affects the heart. In ATTR-CM the amyloid fibrils accumulate in the myocardium.

There are two forms of ATTR-CM, hereditary or variant (hATTR) and wild-type (wtATTR). The hereditary form of ATTR-CM is inherited and caused by a mutation in the transthyretin (TTR) gene, which results in an unstable TTR protein that dissociates and misfolds, forming amyloid, while the wild-type form of ATTR-CM is a result of age-related changes in wild-type TTR stability, and it occurs after age 60.

Transthyretin Amyloid Cardiomyopathy (ATTR-CM) Market Key Facts

  • According to the article by Inserro (2019), the estimated prevalent cases of ATTR-CM in the US is 100,000, and only 1–2% of these are diagnosed.

Transthyretin Amyloid Cardiomyopathy (ATTR-CM) Therapeutics Analysis

The treatment of ATTR-CM has two objectives: to provide medical support, and if possible, to stop or delay amyloid deposition by the use of specific treatments. The supportive therapies that are used to maintain normal heart function include diuretics, beta-blockers, and amiodarone.

In ATTR-CM patients, diuretics are used for the treatment of heart failure. However, because excessive use of diuretics can lead to hypotension and worsen the clinical situation—especially in hereditary ATTR-CM—extreme care must be exercised in its management. In ATTR-CM, impaired diastolic dysfunction and reduced stroke volume lead to compensatory tachycardia. Hence, beta-blockers can be used with care and individualized for each patient. In contrast to heart failure with systolic dysfunction due to other etiologies, there is no evidence in support of a prognostic benefit due to the use of beta-blockers, angiotensin-converting enzyme inhibitors, or angiotensin II receptor antagonists in cardiac amyloidosis. Their use may lead to clinical worsening due to hypotension and low output: a recent publication has reported a worse prognosis in hereditary ATTR-CM and a neutral effect in wild-type ATTR-CM.

Several major pharma and biotech companies are developing therapies for Transthyretin Amyloid Cardiomyopathy (ATTR-CM). Currently, NeuraImmune Therapeutics is leading the therapeutics market with its Transthyretin Amyloid Cardiomyopathy (ATTR-CM) drug candidates in the most advanced stage of clinical development.

Non-Muscle Invasive Bladder Cancer (NMIBC): An Overview

Non-muscle invasive bladder cancer (NMIBC) is cancer of the bladder. NMIBC is a cancer that grows only in the thin tissue on the inside surface of the bladder. With NMIBC, the bladder muscle is not involved, and the tumor does not spread outside the bladder.

There are a number of things that may increase the risk of getting bladder cancer including smoking, working in places that make plastics, paints, leather, and rubber, cancer drugs, radiation to the pelvis, and genes. Blood in the urine is the most common symptom of bladder cancer, which is often painless. Other symptoms of NMIBC include frequent and urgent urination, pain when you pass urine, pain in your lower abdomen, and back pain.

The best treatment for NMIBC is early diagnosis and early care. Treatment will also depend on the stage and the growth of cancer. Treatments for non-muscle invasive bladder cancer include cystoscopic transurethral resection of the bladder tumor (TURBT), intravesical therapy, and surgery. If these options fail to treat the cancer, your doctor may recommend removing the complete bladder.

FDA has approved many drugs for the treatment of bladder cancer but to date there are only two drugs which are being given for the treatment of NMIBC which include KEYTRUDA and ADSTILADRIN, approved in 2020 and 2022, respectively.

Non-Muscle Invasive Bladder Cancer Market Key Facts

  • The total prevalent cases of Bladder Cancer in the US were approximately 800,000 cases in 2022.

  • As per DelveInsight’s estimates, in the US, low-risk NMIBC patients were higher as compared to the high-risk NMIBC patients in 2022.

  • EU4 and the UK had the highest number of diagnosed prevalent cases of NMIBC among the 7MM. In 2022, EU4 and the UK accounted for approximately 250,000 cases.

  • Age-specific incidence of NMIBC cancer increased from 5.52 to 9.09 per 100,000 from 1998 to 2006, as reported in a population-based study on the US population (Nelisen et al., al 2014)

  • Bladder Cancer is more common in males than females.

  • In 2022, total tumor stage-specific diagnosed prevalent cases of NMIBC in Japan for stages Ta, Tis, and T1 were around 50,000, 2,543, and 25,000, respectively.

Non-Muscle Invasive Bladder Cancer (NMIBC) Therapeutics Analysis

NMIBC remains a very challenging disease to treat, with high rates of recurrence and progression associated with current therapies. The high rates of progression and recurrence with current therapies for NMIBC necessitate lifelong active surveillance, making bladder cancer the most expensive cancer to treat from diagnosis to death, as well as driving the need for the development of new therapies in patients with NMIBC.

The current treatment regimen includes KEYTRUDA (pembrolizumab), ADSTILADRIN, surgery, intravesical immunotherapy (BCG), and intravesical chemotherapy. Intermediate- or high-risk NMIBC is generally treated with transurethral resection of bladder tumor (TURBT), followed by adjuvant BCG immunotherapy which is the gold standard treatment for reducing tumor recurrence rates and preventing subsequent stage progression. Stage 0 bladder cancer is most often treated with TURBT with fulguration followed by intravesical therapy within 24 h. Sometimes no further treatment is needed. Cystoscopy is then done every 3–6 months to watch for signs that cancer has come back. Valrubicin might be used if BCG stops working. However, not all experts agree on this treatment.

Multiple Sclerosis: An Overview

Multiple Sclerosis (MS) is an unpredictable, autoimmune disease that affects the central nervous system. In Multiple Sclerosis, the body’s own immune system produces cells, and proteins (antibodies) to attack the fatty substance, myelin, which is responsible for protecting the nerve fibers. This is often characterized by relapsing episodes of neurologic impairment followed by remissions.

The damaged myelin forms scar tissue (sclerosis). Often the nerve fiber is also damaged. When any part of the myelin sheath or nerve fiber is damaged or destroyed, nerve impulses traveling to and from the brain and spinal cord are distorted or interrupted. People with Multiple Sclerosis generally develop symptoms in their late 20s, experiencing visual and sensory disturbances, limb weakness, gait problems, and bladder and bowel syndromes. Initially, they may have partial recovery, but over time they develop progressive disability.

Multiple Sclerosis-affected individuals may experience a variety of symptoms, physically, emotionally, mentally, and psychologically. The severity and types of symptoms differ among individuals; thus, symptoms differ among different people.

With the continuous efforts in research and development, various companies are developing therapies, NeuroVax (Immune Response BioPharma), CNM-Au8 (Clene Nanomedicine), Tolebrutinib (SAR442168) (Sanofi), Evobrutinib/M2951 (Merck Healthcare KGaA), and many others are expected to enter the 7MM market by 2032.

Burns: An Overview

Burn injuries are underappreciated injuries that are associated with substantial morbidity and mortality. Burn injuries, particularly severe burns, are accompanied by an immune and inflammatory response, metabolic changes, and distributive shock that can be challenging to manage and can lead to multiple organ failures. Of great importance is that the injury affects not only physical health but also the patient’s mental health and quality of life.

Burn injuries are underappreciated trauma that can affect anyone, anytime and anywhere. The injuries can be caused by friction, cold, heat, radiation, chemical, or electric sources, but most burn injuries are caused by heat from hot liquids, solids, or fire. Although all burn injuries involve tissue destruction due to energy transfer, different causes can be associated with different physiological and pathophysiological responses.

In addition to determining the cause of a burn injury, it is imperative to classify the injury according to its severity –its depth and size. Burns that affect the uppermost layer of the skin (epidermis only) are classed as superficial (first-degree) burns; the skin becomes red, and the pain experienced is limited in duration. Superficial partial-thickness (second-degree) burns (formerly known as 2A burns) are painful, weep, require dressing and wound care, and may scar but do not require surgery. Deep partial-thickness (second-degree) burns (formerly known as 2B burns) are less painful due to the partial destruction of the pain receptors, which are drier, require surgery, and will scar. A full-thickness (third-degree) burn extends through the full dermis and is not typically painful owing to damage to the nerve endings and requires protection from becoming infected and, unless minimal, surgical management.

Burns Market Key Facts

  • The market size for Burns injury was USD 827 million in 2021 in the United States.

  • The Burns market size in the EU5 was USD 491 million in 2021. EU5 accounted for nearly 32% of the total 7MM market size.

  • Among the EU5 countries, the United Kingdom holds the highest market size of USD 160 million, followed by France (USD 103 million) and Germany (USD 85 million). Spain holds the least market size of USD 71 million among the EU5 countries in 2021.

  • The Burns market size in Japan was USD 222 million in 2021. Japan accounted for 14% of the total 7MM market size.

  • In 2021, the total incident population of burn injuries that required medical treatment in seven major markets was reported to be 1,864,208.

  • The estimates suggest the United States had the highest incident population of burn injuries that required medical treatment, with 661,900 cases in 2021.

  • According to our analysis, after fire/flame (41%), scald was the foremost contributing cause of burn injuries in the US, occupying 31.4% of total incident cases of treatment-taking burn injuries.

  • According to our assessments for the United States, the estimated cases of hospitalized burn patients in 2021 were 76,383.

  • Japan had 328,255 incident cases of burn injuries requiring medical treatment in 2021, the second-highest of all other countries in 7MM after the US.

Burns Therapeutics Analysis

In the current market scenario, the standard treatment for skin burns involves medications, wound dressings, therapy, and surgery. The treatment goals are to control pain, remove dead tissue, prevent infection, reduce scarring risk, and regain function. The treatment depends on the severity of the burn.

Medical treatment includes water-based treatments (Ultrasound mist therapy to clean and stimulate the wound tissue), fluids to prevent dehydration (intravenous (IV) fluids to prevent dehydration and organ failure), pain and anxiety medications (morphine and anti-anxiety medications), creams and ointments (bacitracin and silver sulfadiazine), dressings (wound dressings), and common topical antimicrobials such as silver sulfadiazine (SSD), cerium plus SSD, silver nitrate, mafenide, bacitracin, and Xeroform.

Additionally, because children have limited glycogen stores, they should be given weight-based maintenance intravenous fluid in the form of 5% dextrose in 4.5% normal saline in addition to burning resuscitation fluid. Elderly patients have a unique acute phase response after burn injury, characterized by decreased organ perfusion and oxygenation.

To further improve the treatment outlook, several major pharma and biotech companies are developing therapies for Burns. Currently, NeoMatrix Therapeutics is leading the therapeutics market with its Burns drug candidates in the most advanced stage of clinical development.

Thrombocytopenia: An Overview

Thrombocytopenia is a medical condition in which the platelet count in the blood falls (<150,000/µL of circulating blood) below the normal value. The etiology of thrombocytopenia is vast as many conditions may lead to low platelet count in the blood. The most common etiologies are heparin-induced thrombocytopenia, chemotherapy-induced thrombocytopenia, thrombocytopenia associated with chronic liver disease, immune thrombocytopenia (ITP), and thrombotic thrombocytopenic purpura (TTP).

Thrombocytopenia often improves when its underlying cause is treated. People who inherit the condition do not usually need treatment. If a reaction to a medicine is causing a low platelet count, the doctor may prescribe another medicine. Most people recover after the initial medicine has been stopped. For heparin-induced thrombocytopenia (HIT), stopping the heparin is not enough. Often, the patients need another medicine to prevent blood clotting. If the immune system is causing a low platelet count, the doctor may prescribe medicines to suppress the immune system.

Thrombocytopenia can result from decreased platelet production, increased platelet consumption, or sequestration. A systematic approach should be used to evaluate incidental thrombocytopenia. Physicians should inquire about easy bruising or petechiae, melena, rashes, fevers, and bleeding during the patient’s history. They also should inquire about medication use, immunizations, recent travel, transfusion history, family history, and medical history. A history of acute and chronic alcohol use should be obtained. Any recent hospitalization or heparin exposure should raise the possibility of heparin-induced thrombocytopenia.

Thrombocytopenia Therapeutics Analysis

Corticosteroids are the most commonly used first-line therapy for ITP. Treatment is initiated with prednisolone or prednisone (1–2 mg/kg per day, p.o., single or divided doses). In general, corticosteroids are initially effective. A study comparing high- and low-dose corticosteroids in adult patients found that low-dose therapy was initially more effective, although long-term effects have not been determined.

The approved therapies for treating thrombocytopenia in the US include MULPLETA, DOPTELET, TAVALISSE, PROMACTA, NPLATE, CABLIVI, GAMMAPLEX, OCTAGAM, RHOPHYLAC, and PRIVIGEN. These therapies are approved for various patient groups; for example, MULPLETA and DOPTELET are approved for thrombocytopenia in chronic liver disease, PROMACTA is approved for chronic immune (idiopathic) thrombocytopenic purpura (ITP).

The approved therapies in the EU include MULPLETA/MULPLEO (lusutrombopag), DOPTELET (avatrombopag), TAVALISSE/TAVLESSE (fostamatinib), PROMACTA/REVOLADE (eltrombopag), NPLATE/ROMIPLATE (romiplostim), and CABLIVI (caplacizumabum).

OCTAGAM, RHOPHYLAC, and PRIVIGEN are the immunoglobulins approved in the EU to treat immune thrombocytopenic purpura.
